Current members:
Please fill out the form if you
have any information that
has changed, such as a new telephone number or mailing
address. If you are a current member who needs to update your information,
you do not need to complete a whole new form, just fill in the new
information.
New members:
Please fill out both
pages of the Membership Application Form.
Please
note: It is important to have the site map on the second page
show where the septic system is located so emergency services does not drive
over your system if they need to go around the cabin to fight a fire or for
any other
purpose. Also, if you have a propane tank, please let us know if it is an above or below ground
tank. Emergency services may need to shut it off.
Cost: It is free to join DPNW. However, members are encouraged to purchase products from our Fun Begins at Crescent Lake, Oregon© line to support DPNW. Proceeds from purchases help fund our organization.
